Chronic Superficial Keratitis, widely known as CSK or pannus, is a progressive, bilateral, immune-mediated inflammatory disease that affects the cornea of dogs. The condition is characterized by the infiltration of blood vessels, inflammatory cells, and pigment into the normally clear corneal tissue, leading to varying degrees of opacity and, if untreated, potential blindness. First described by Uberreiter in the 1960s, CSK has since become one of the most commonly recognized immune-mediated ocular conditions in veterinary ophthalmology.
The disease typically begins at the lateral or ventrolateral limbus, the junction where the cornea meets the white of the eye, and progresses centrally across the corneal surface over time. The advancing edge of the lesion is marked by a characteristic pink or red vascular infiltrate, followed by a zone of granulation tissue that eventually becomes pigmented. As the disease progresses, fibrous tissue and melanin deposition replace the normal transparent corneal stroma, resulting in a brownish-black opacification that can significantly impair vision.
CSK is fundamentally an autoimmune process in which the dog's own immune system inappropriately targets the corneal tissue. The exact triggering mechanism remains incompletely understood, but it is believed to involve a complex interplay between genetic predisposition, environmental factors, and immune dysregulation. The condition is not painful in its early stages, which can lead to delayed recognition by owners, but advanced cases may develop secondary complications including corneal ulceration that can cause discomfort.
Understanding CSK as a lifelong, manageable condition rather than a curable disease is essential for both veterinary professionals and dog owners. While treatment can effectively control the progression and even reverse some of the corneal changes, the underlying immune-mediated process cannot be permanently eliminated. Lifelong therapy and regular monitoring are necessary to maintain corneal clarity and preserve vision in affected dogs.
