Pannus, medically known as chronic superficial keratitis, is an immune-mediated inflammatory disease affecting the cornea of dogs that, without treatment, progressively leads to vision impairment and potential blindness. This condition occurs when the immune system inappropriately targets corneal tissue, causing inflammation, blood vessel growth, pigmentation, and scarring that gradually obscures the normally transparent cornea. Pannus is particularly common in certain breeds, with German Shepherds showing such high prevalence that the condition has historically been called German Shepherd pannus. While not curable, pannus is highly manageable with appropriate lifelong treatment, and affected dogs can maintain good vision and quality of life with proper care.
The underlying cause of pannus involves a complex interaction between genetic predisposition, immune dysregulation, and environmental triggers, particularly ultraviolet light exposure. In affected dogs, immune cells infiltrate the cornea, initiating an inflammatory cascade that damages corneal tissue and stimulates compensatory responses including blood vessel growth and pigment deposition. The disease typically begins at the lateral or temporal aspect of the cornea, near the junction with the white of the eye, and progressively extends centrally toward the visual axis. The characteristic appearance includes a vascularized, pigmented, granular tissue that advances across the corneal surface. A related condition called plasmacytic conjunctivitis often accompanies pannus and affects the third eyelid.
The impact of pannus on affected dogs depends heavily on the stage at which treatment is initiated and the consistency of ongoing therapy. In early stages, dogs may show minimal clinical signs, with changes detectable only on close examination. As the disease progresses without treatment, corneal opacity increases, causing visual impairment that worsens over time. Advanced untreated cases can result in complete corneal coverage with opaque, vascularized tissue and functional blindness. The condition affects both eyes, though often asymmetrically. Dogs experience no significant pain from pannus itself, which can paradoxically lead to delayed diagnosis as owners may not notice the gradual changes until substantial vision loss has occurred.
Pannus requires lifelong management rather than cure, but the prognosis for maintaining functional vision is good with consistent treatment. Topical immunosuppressive medications suppress the immune attack on corneal tissue, halting progression and often producing partial regression of existing changes. Treatment must continue indefinitely, as discontinuation allows disease progression to resume. Environmental management, particularly reducing ultraviolet light exposure, complements medical therapy. Regular veterinary monitoring ensures treatment remains effective and allows adjustment of medication regimens as needed. The key message for owners of affected dogs is that commitment to lifelong treatment is essential but rewarding, as it preserves vision and quality of life.
