Pigmentary keratitis is a progressive condition in dogs characterized by the deposition of brown or black melanin pigment onto the normally clear cornea, potentially leading to visual impairment if left untreated. This condition is particularly prevalent in brachycephalic breeds, with Pugs showing such high frequency that it is sometimes informally called Pug keratitis. The pigmentation represents the cornea's response to chronic irritation, inflammation, or hypoxia, with melanin-containing cells migrating onto the corneal surface from the limbus. While the condition develops gradually over months to years, without treatment the pigment can eventually cover enough of the cornea to significantly obstruct vision.
The causes of pigmentary keratitis are multifactorial, typically involving chronic corneal irritation from anatomical abnormalities common in predisposed breeds. Brachycephalic dogs possess prominent, protruding eyes that experience increased exposure to environmental irritants and reduced protection from eyelid coverage. Many affected dogs have some degree of lagophthalmos, an inability to fully close the eyelids, leading to chronic exposure of the central cornea. Concurrent conditions including dry eye, entropion, distichiasis, and nasal fold trichiasis contribute to ongoing corneal irritation that stimulates pigment deposition. The medial, or inner, aspect of the cornea is typically affected first, corresponding to areas of greatest exposure or trichiasis contact.
The impact of pigmentary keratitis on affected dogs depends on the extent of pigment deposition and the rate of progression. In early stages, owners may notice a subtle brownish discoloration at the inner corner of their dog's eye without apparent discomfort or visual problems. As pigmentation spreads across the cornea toward the visual axis, vision becomes increasingly obstructed. The condition itself is not painful, which can lead to delayed recognition by owners. Both eyes are typically affected, though often asymmetrically. Quality of life impacts range from minimal in early cases to significant vision impairment in advanced, untreated cases. Early recognition and treatment offer the best chance of preserving clear vision.
Pigmentary keratitis is manageable with appropriate treatment that addresses both the pigmentation itself and the underlying causes. Topical immunosuppressive medications can slow or halt pigment progression and may reduce existing pigmentation in some cases. Equally important is addressing contributing factors such as dry eye, eyelid abnormalities, and nasal fold irritation. Surgical correction of anatomical problems may be necessary for optimal long-term control. The prognosis is generally good when treatment begins early and contributing causes are effectively managed. Owners of brachycephalic breeds should be aware of this condition and monitor their dogs' eyes for early signs that prompt veterinary evaluation.
