Keratoconjunctivitis sicca, commonly known as dry eye or KCS, is a chronic condition in dogs characterized by inadequate tear production that fails to properly lubricate and protect the ocular surface. The tear film normally performs essential functions including nourishing the cornea, washing away debris and pathogens, providing a smooth optical surface for clear vision, and delivering antibodies and other protective substances. When tear production is deficient, the cornea and conjunctiva become dry, irritated, and susceptible to damage, leading to discomfort, vision impairment, and potentially sight-threatening complications.
The underlying cause of dry eye in dogs is most commonly immune-mediated destruction of the lacrimal glands, the tissues responsible for producing the aqueous component of tears. This autoimmune process progressively damages the glands, reducing their capacity to generate adequate tears. Less commonly, dry eye results from toxic effects of certain medications, neurological conditions affecting tear gland innervation, congenital absence or malformation of tear glands, or trauma to the glands. Understanding the cause helps guide treatment selection and predict response to therapy.
Dry eye significantly impacts affected dogs through chronic discomfort and progressive ocular surface damage. The characteristic thick, mucoid discharge results from inadequate liquid tears to properly dilute and distribute the mucin component of the tear film. Corneal changes including blood vessel growth, pigmentation, and scarring progressively impair vision if the condition is not adequately treated. The constant irritation causes squinting, rubbing, and behavioral evidence of ocular discomfort. Quality of life is substantially diminished when dry eye is left untreated or inadequately managed.
With appropriate treatment, most dogs with dry eye can achieve good comfort and maintain useful vision throughout their lives. The development of cyclosporine ophthalmic preparations revolutionized treatment of this condition by addressing the underlying immune-mediated damage rather than merely supplementing tears. Many dogs show significant improvement in tear production within weeks of starting therapy, though lifelong treatment is typically required. Early diagnosis and consistent treatment provide the best outcomes, while delayed or inconsistent therapy allows progressive damage that may become irreversible.
