Benign prostatic hyperplasia, commonly abbreviated as BPH, is a non-cancerous enlargement of the prostate gland that occurs in intact male dogs as a natural consequence of chronic testosterone exposure. The prostate is a bilobed gland that encircles the proximal urethra just caudal to the urinary bladder in male dogs. Its primary function is to produce a significant portion of the seminal fluid that nourishes and transports spermatozoa during ejaculation. Under the sustained influence of testosterone and its more potent intraprostatic metabolite dihydrotestosterone, the glandular and stromal cells of the prostate undergo progressive proliferation, resulting in symmetric enlargement of the organ.
BPH is by far the most common prostatic condition in dogs and is considered an almost inevitable consequence of aging in intact males. Histological evidence of prostatic hyperplasia can be detected in the majority of intact male dogs by the age of five, and the prevalence approaches virtually all intact males by eight to ten years of age. Despite this near-universal occurrence, only a fraction of affected dogs develop clinical signs significant enough to require treatment, as the degree of enlargement and its impact on surrounding structures varies considerably among individuals.
The condition develops gradually over months to years and progresses through recognizable stages. Early BPH is characterized by glandular hyperplasia, in which the secretory epithelial cells of the prostate proliferate. As the condition advances, a cystic component often develops, termed cystic hyperplasia, in which fluid-filled cysts of varying sizes form within the prostatic parenchyma. In some dogs, the enlarged prostate may also develop areas of squamous metaplasia, particularly under the influence of exogenous or endogenous estrogens.
It is clinically important to distinguish BPH from other prostatic conditions including prostatitis, prostatic abscessation, prostatic cysts, and prostatic neoplasia, as the treatment approaches and prognoses differ substantially. While BPH itself is benign, an enlarged prostate may be predisposed to secondary complications such as bacterial prostatitis, and the clinical signs of various prostatic diseases can overlap considerably, necessitating thorough diagnostic evaluation.
