Benign prostatic hyperplasia, commonly abbreviated as BPH, is a condition affecting intact male dogs in which the prostate gland undergoes progressive enlargement under the influence of testosterone and other hormones over time. This non-cancerous enlargement represents the most common prostatic disorder in dogs, affecting virtually all intact male dogs to some degree by the time they reach middle to advanced age. The prostate gland surrounds the urethra at its junction with the bladder, meaning that significant enlargement can compress urinary and digestive structures, potentially causing symptoms ranging from subtle elimination difficulties to significant discomfort.
The development of BPH occurs through hormone-mediated cellular proliferation within the prostate gland. Testosterone produced by the testicles is converted to dihydrotestosterone within prostatic tissue, stimulating growth of both glandular and stromal components of the prostate. This hormone-dependent growth process continues throughout the lives of intact males, with prostate size progressively increasing from sexual maturity onward. By five to six years of age, most intact male dogs show evidence of prostatic enlargement on examination, though only a proportion develop clinical symptoms significant enough to require intervention.
The impact of BPH on an individual dog's health and wellbeing depends primarily on the degree of enlargement and whether compression of surrounding structures causes functional problems. Many dogs with significantly enlarged prostates remain asymptomatic throughout their lives, while others develop difficulties with urination, defecation, or both due to mechanical compression of the urethra and colon. When symptoms develop, they typically progress gradually, allowing owners and veterinarians time to recognize and address the problem before serious complications occur. Understanding that prostatic enlargement is hormonally driven helps explain why treatment approaches focus on removing hormonal stimulation.
The excellent treatability of BPH represents one of the most positive aspects of this condition. Surgical castration removes the hormonal driver and results in rapid, dramatic prostate shrinkage in most dogs, typically resolving symptoms within weeks. Medical management options exist for dogs where surgery is not desired or feasible, providing symptom control through alternative approaches. Early recognition and appropriate treatment prevents progression to complications including prostatic cysts, infections, and urinary obstruction.
