Prostatitis is inflammation of the prostate gland, most commonly caused by bacterial infection in male dogs. This condition represents one of the most significant prostatic diseases affecting intact male dogs and can range from acute, severe illness requiring emergency treatment to chronic, smoldering infection that causes subtle ongoing symptoms. The prostate gland, located at the base of the bladder surrounding the urethra, becomes colonized by bacteria that trigger inflammatory responses, leading to pain, dysfunction, and systemic illness.
Bacterial prostatitis typically develops through ascending infection, where bacteria from the urinary tract travel through the urethra to reach and colonize prostatic tissue. The prostate gland in intact male dogs provides a favorable environment for bacterial establishment, particularly when benign prostatic hyperplasia has altered normal glandular architecture. Once established, infection can persist and cause ongoing damage if not appropriately treated. Less commonly, bacteria may reach the prostate through the bloodstream from distant infection sites.
Acute prostatitis presents dramatically with fever, pain, and obvious systemic illness. Dogs may show reluctance to move, difficulty urinating, and significant discomfort. This form of prostatitis requires prompt veterinary attention as it can progress rapidly to more serious complications including prostatic abscess formation, sepsis, and death. The severity of acute prostatitis demands aggressive treatment with appropriate antibiotics and supportive care.
Chronic prostatitis develops when acute infection is inadequately treated or when low-grade infection persists without causing obvious acute illness. This form is more insidious, with dogs showing intermittent mild symptoms that may wax and wane over time. Chronic prostatitis can be challenging to diagnose and treat, often requiring prolonged antibiotic courses. Left untreated, chronic prostatitis serves as a nidus for recurrent urinary tract infections and may eventually lead to prostatic abscess formation or other serious complications.
