Paraprostatic cysts are fluid-filled sacs that develop adjacent to or attached to the prostate gland in male dogs. Unlike intraprostatic cysts, which form within the parenchyma of the prostate itself, paraprostatic cysts extend beyond the prostatic capsule and can grow to considerable size within the pelvic and abdominal cavities. These structures may remain connected to the prostate by a stalk or pedicle, or they may become largely independent, maintaining only a thin attachment to the gland or surrounding tissues.
The origin of paraprostatic cysts has been a subject of debate in veterinary medicine. One prevailing theory suggests that they arise from remnants of the uterus masculinus, a vestigial Mullerian duct structure that persists in male dogs. Another theory proposes that they develop as extensions of intraprostatic retention cysts that gradually enlarge and protrude beyond the prostatic capsule. In some cases, both mechanisms may contribute to cyst formation, and the distinction may be largely academic when the clinical presentation and treatment approach are similar.
These cysts can range dramatically in size, from small incidental findings detected during imaging for unrelated conditions to massive fluid-filled structures that occupy a significant portion of the caudal abdomen. Cysts containing several liters of fluid have been reported, and the progressive enlargement of these structures is responsible for the compressive symptoms that typically prompt clinical evaluation. The fluid within paraprostatic cysts is usually serous or serosanguineous, though secondary infection can alter the character of the fluid significantly.
Paraprostatic cysts are most commonly diagnosed in intact male dogs, reflecting the hormone-dependent nature of prostatic disease in canines. The condition is relatively uncommon compared to other forms of prostatic disease such as benign prostatic hyperplasia and prostatitis, but it represents an important differential diagnosis in male dogs presenting with signs referable to the caudal abdomen or pelvic region.
