Cystic endometrial hyperplasia (CEH) is a condition in which the lining of the uterus, known as the endometrium, undergoes abnormal thickening and develops fluid-filled cysts. This condition occurs almost exclusively in intact (unspayed) female dogs and is driven by the cumulative effects of progesterone during repeated estrous cycles. Over time, the endometrial tissue becomes increasingly disorganized and prone to secondary complications, most notably the development of pyometra, a life-threatening uterine infection.
The condition is part of a well-recognized clinical spectrum known as the cystic endometrial hyperplasia-pyometra complex. In this progression, the initial hormonal changes cause the uterine lining to proliferate abnormally, creating an environment that is highly susceptible to bacterial colonization. The cystic changes reduce the uterus's natural defense mechanisms and impair its ability to clear bacteria, setting the stage for infection.
CEH is considered one of the most common reproductive disorders in dogs. While it can occur in females of any age after their first heat cycle, the incidence increases significantly in middle-aged and older intact females. Dogs that have never been bred are at particularly elevated risk, as are those that have been treated with exogenous hormones such as progesterone-based drugs or estrogen compounds used for mismating injections.
Understanding CEH is critical for dog owners who maintain intact females, whether for breeding purposes or personal preference. Early recognition of the condition allows for timely intervention that can preserve fertility in breeding animals or prevent the serious and potentially fatal consequences of pyometra in non-breeding dogs.
