Retained deciduous teeth occur when a dog's primary teeth, commonly called baby teeth or milk teeth, fail to fall out naturally as the permanent adult teeth erupt. In normal dental development, the roots of deciduous teeth are gradually resorbed as the permanent teeth push upward through the gums, causing the baby teeth to loosen and eventually exfoliate. When this process fails, both the deciduous tooth and its permanent replacement occupy the mouth simultaneously, creating a condition often referred to as persistent deciduous teeth or double teeth. This developmental abnormality is remarkably common in dogs, particularly affecting small and toy breeds, with some studies suggesting prevalence rates as high as 12 percent in certain susceptible populations.
The condition develops when the permanent tooth erupts in an abnormal position that does not contact and stimulate resorption of the deciduous tooth root. Instead of emerging directly beneath the baby tooth and pushing it out, the permanent tooth grows alongside it, leaving both teeth present in the mouth. The most commonly retained deciduous teeth are the upper canines, followed by the lower canines and upper incisors, though any deciduous tooth can potentially be retained. The failure of normal exfoliation may result from various factors including genetic predisposition, abnormal tooth positioning, or insufficient resorption stimulus.
The significance of retained deciduous teeth extends well beyond cosmetic concerns. When both teeth occupy space intended for one, crowding occurs that disrupts normal dental alignment and occlusion. The narrow space between the retained deciduous tooth and the permanent tooth traps food particles, hair, and debris, creating an ideal environment for plaque accumulation and bacterial growth. This leads to accelerated tartar formation, gingivitis, periodontal disease, and potential tooth loss affecting the permanent dentition. Additionally, persistent deciduous teeth can cause the permanent teeth to erupt in abnormal positions, leading to malocclusion that may cause trauma to the soft tissues of the mouth or opposing teeth.
Fortuante, retained deciduous teeth are readily treatable through surgical extraction, and early intervention produces excellent outcomes. Veterinarians typically recommend extraction as soon as the condition is identified, ideally by the time the puppy reaches six to seven months of age. The longer deciduous teeth remain, the greater the risk of permanent damage to the developing adult dentition and supporting structures. With prompt treatment, most dogs experience complete resolution with normal permanent tooth positioning and function. Regular veterinary examinations during the teething period are essential for early detection, allowing timely intervention that prevents long-term complications.
