Heart disease, particularly endocarditis (infection of the heart valves), has been linked to chronic oral infection in dogs, with bacteria from periodontal disease potentially seeding heart valve tissue. Dogs with these concurrent conditions require careful pre-surgical workup and may need modified treatment approaches.

Several conditions produce symptoms that can overlap with or be confused for dental disease, making accurate diagnosis important. Oral tumors including melanoma, squamous cell carcinoma, and fibrosarcoma can cause bleeding, loose teeth, facial swelling, and difficulty eating similar to advanced periodontal disease. Jaw fractures, particularly pathologic fractures occurring through bone weakened by periodontal disease, may present with sudden pain, swelling, and inability to close the mouth normally. Masticatory muscle myositis, an autoimmune condition affecting the chewing muscles, causes jaw pain and difficulty eating that might be attributed to dental pain. Foreign bodies lodged between teeth or in the gums can cause localized inflammation mimicking periodontal disease at a specific site.

Complications arising from untreated or advanced dental disease extend the condition's impact beyond the oral cavity. Oronasal fistulas develop when infection destroys the bone between upper tooth roots and the nasal cavity, creating an abnormal connection that causes chronic nasal discharge and sneezing. Jaw fractures can occur spontaneously in dogs with severe bone loss, particularly in small breeds with thin mandibles. Eye infections or abscesses may result from spread of infection from upper premolar or molar roots located near the orbit. Systemic spread of bacteria can affect distant organs including heart valves, kidneys, and liver, contributing to or exacerbating dysfunction of these organs. Chronic pain and infection may suppress the immune system and contribute to other health problems.
