Ectopic cilia represent one of the most painful and potentially vision-threatening eyelash abnormalities affecting dogs. Unlike distichiasis where extra lashes emerge from the lid margin, ectopic cilia are aberrant hairs that grow through the palpebral conjunctiva, the inner surface of the eyelid that directly contacts the eye. This abnormal positioning places the hair in direct contact with the cornea, causing intense pain and progressive corneal damage with every blink. Ectopic cilia most commonly occur in young dogs, typically presenting before two years of age, and show strong breed predisposition in certain populations.
The developmental origin of ectopic cilia involves misdirected hair follicle formation within the meibomian glands during embryonic eyelid development. Instead of producing oil as meibomian glands should, or emerging from the lid margin like normal lashes or even distichia, these follicles grow through the inner eyelid surface. The resulting hair directly abrades the cornea with each blink, creating a painful cycle of damage, inflammation, and attempted healing. A single ectopic cilium can cause severe clinical disease, though multiple cilia may be present.
The clinical significance of ectopic cilia cannot be overstated due to their potential to cause serious corneal damage. The constant friction between the abnormal hair and the corneal surface produces painful erosions and ulcers that may progress to deep stromal involvement. Dogs with ectopic cilia experience significant discomfort, often showing dramatic clinical signs including severe squinting, excessive tearing, and obvious distress. Without identification and treatment, corneal scarring and potentially vision loss may result. The location of the lesion often correlates with the position of the ectopic cilium, providing a diagnostic clue.
Successful treatment of ectopic cilia requires surgical excision of the offending hair follicle, as the hair will continuously regrow if the follicle remains intact. Veterinary ophthalmologists perform this procedure using magnification and specialized techniques to ensure complete removal. When diagnosed and treated promptly, dogs with ectopic cilia have excellent prognoses for resolution of clinical signs and preservation of vision. The hereditary component means that affected dogs and their close relatives should not be used for breeding, helping reduce prevalence in predisposed breeds.
