Pemphigus foliaceus is the most common autoimmune skin disease affecting dogs, characterized by the formation of pustules, crusts, and erosions on the skin surface. This condition occurs when the immune system produces antibodies that attack desmoglein, a protein that acts as the glue holding skin cells together in the superficial layers of the epidermis. The resulting loss of cell adhesion causes the skin to blister and form the characteristic pustular lesions that define this disease. Pemphigus foliaceus typically affects middle-aged dogs but can occur at any age.
The mechanism underlying pemphigus foliaceus involves autoantibodies, specifically targeting desmoglein-1, a component of the desmosomes that connect adjacent skin cells. When these antibodies bind to desmoglein, they trigger a cascade of events that weakens the connections between cells in the upper layers of skin. This process, called acantholysis, results in separation of cells and formation of fluid-filled spaces that present as pustules. The superficial nature of the blistering distinguishes pemphigus foliaceus from deeper blistering disorders and accounts for the typical crusting appearance of lesions.
Dogs with pemphigus foliaceus typically develop lesions first on the face, ears, and footpads, though the condition can spread to affect large areas of the body over time. The initial pustules are fragile and rupture quickly, leaving behind characteristic thick, adherent crusts and scales. Lesions often appear symmetric and may wax and wane in severity without treatment. The footpads frequently become thickened, cracked, and painful, causing lameness in many affected dogs. Secondary bacterial infections commonly complicate the condition due to disruption of the normal skin barrier.
While pemphigus foliaceus is a serious condition requiring lifelong management, most dogs can achieve remission with appropriate immunosuppressive treatment. The goal of therapy is to suppress the autoimmune attack on the skin while minimizing side effects from medications. Many dogs respond well to treatment and can maintain good quality of life with ongoing medication and monitoring. Working with a veterinarian or veterinary dermatologist experienced in autoimmune skin diseases provides the best chance for successful long-term management.
