Canine pemphigus is a group of autoimmune skin diseases characterized by the production of autoantibodies directed against structural proteins within the epidermis. These autoantibodies target desmosomal adhesion molecules, which are the proteins responsible for binding keratinocytes together within the skin. When these intercellular connections are disrupted, the keratinocytes lose cohesion in a process known as acantholysis, leading to the formation of intraepidermal clefts, vesicles, blisters, and ultimately erosions and ulcers on the skin surface.
The pemphigus complex in dogs includes several distinct variants, each defined by the depth of epidermal involvement and the specific target antigens attacked by the immune system. Pemphigus foliaceus is by far the most common form in dogs, accounting for the vast majority of diagnosed cases. Pemphigus vulgaris, pemphigus vegetans, pemphigus erythematosus, and paraneoplastic pemphigus are less frequently encountered. Each variant has characteristic clinical features, histological patterns, and prognostic implications that distinguish it from the others.
The autoimmune nature of pemphigus means that the dog's own immune system has lost tolerance to self-antigens and is actively attacking the structural integrity of the skin. This immunological misdirection results in a chronic and often relapsing disease course that requires sustained immunosuppressive therapy to control. Without treatment, the progressive loss of skin barrier function leads to secondary bacterial infections, fluid and protein loss, pain, and significant deterioration in quality of life.
Pemphigus can occur in dogs of any age, breed, or sex, although certain breeds demonstrate higher susceptibility to specific forms of the disease. The condition may arise spontaneously, or it may be triggered by environmental factors such as ultraviolet light exposure, drug reactions, or chronic skin disease. Understanding the immunopathological basis of pemphigus is essential for guiding appropriate diagnostic evaluation and implementing effective long-term treatment strategies.
