Mitral valve dysplasia is a congenital heart defect in cats characterized by malformation of the mitral valve, the valve that separates the left atrium from the left ventricle. This developmental abnormality results in a valve that cannot function properly, typically causing mitral regurgitation where blood leaks backward from the ventricle into the atrium during each heartbeat. The condition is recognized as one of the more common congenital cardiac abnormalities in cats and can range from mild, causing minimal clinical impact, to severe, leading to heart failure at a young age.
The mitral valve normally consists of two leaflets attached to the heart wall by fibrous cords called chordae tendinae, which connect to muscular projections called papillary muscles. In cats with mitral valve dysplasia, various components of this complex apparatus may be malformed. The valve leaflets may be thickened, shortened, or abnormally shaped. The chordae tendinae may be too short, too long, fused, or absent. The papillary muscles may be malpositioned or abnormal in structure. Any combination of these defects can occur, resulting in a valve that fails to close properly and allows blood to regurgitate into the left atrium.
The impact of mitral valve dysplasia on feline health depends largely on the severity of the malformation and resulting regurgitation. Mild cases may produce a heart murmur but cause no clinical symptoms, allowing affected cats to live normal lives without treatment. Moderate to severe cases lead to progressive volume overload of the left heart chambers, as the heart must pump extra blood to compensate for the portion that leaks backward. Over time, this overload causes enlargement of the left atrium and left ventricle, potentially progressing to congestive heart failure. The enlarged left atrium also predisposes to arrhythmias and blood clot formation.
Treatment approaches for mitral valve dysplasia range from monitoring without intervention for mild cases to medical management of heart failure for more severe disease. Surgical repair or replacement of the malformed valve, while performed in humans, remains rare in veterinary medicine due to technical challenges and limited availability. With appropriate management, many cats with mitral valve dysplasia can enjoy good quality of life, though the long-term prognosis depends heavily on disease severity and response to treatment. Early detection through recognition of heart murmurs and subsequent diagnostic workup allows for timely intervention and optimal outcomes.
