The condition may involve the valve itself, the area just below the valve in the left ventricular outflow tract, or both regions. The development of aortic stenosis occurs during fetal cardiac formation when structures of the left ventricular outflow tract fail to develop normally. The obstruction increases the workload of the left ventricle, which must generate higher pressures to eject blood past the narrowed area. This increased workload leads to compensatory thickening of the left ventricular wall, a response called concentric hypertrophy. While initially adaptive, this hypertrophy can eventually become problematic, affecting ventricular function and predisposing to arrhythmias. The turbulent blood flow created by the stenosis produces a heart murmur that is typically the first indication of the condition.
The clinical significance of aortic stenosis depends on the severity of obstruction and the resulting cardiac adaptations. Cats with mild stenosis may live normal lives without symptoms, with the condition discovered incidentally during routine examination. Moderate stenosis causes progressive left ventricular changes that may eventually produce symptoms including exercise intolerance and weakness. Severe aortic stenosis can cause significant clinical signs including syncope, congestive heart failure, and carries risk of sudden death from arrhythmias or inadequate cardiac output during exertion. Early detection and appropriate monitoring are essential for optimal outcomes.
Treatment for aortic stenosis in cats focuses on managing the cardiac consequences of the obstruction rather than eliminating the obstruction itself. Medical therapy addresses symptoms and secondary complications including heart failure and arrhythmias. Surgical correction or catheter-based interventions are occasionally performed but are technically challenging and not widely available for cats. Regular monitoring allows detection of disease progression and timely treatment adjustments. Working with a veterinarian experienced in cardiac disease or a veterinary cardiologist ensures appropriate management and optimal outcomes for affected cats.
