Atrial fibrillation is a serious cardiac arrhythmia characterized by rapid, chaotic electrical activity in the heart's upper chambers (atria), resulting in an irregular and often abnormally fast heartbeat. In cats, this condition is relatively uncommon compared to dogs, but when it occurs, it almost always indicates the presence of significant underlying heart disease. During atrial fibrillation, the atria quiver ineffectively rather than contracting in a coordinated manner, which disrupts normal blood flow through the heart and can significantly compromise cardiac function. The irregular ventricular response that results from the chaotic atrial activity produces the characteristically irregular pulse that veterinarians detect during physical examination.
The development of atrial fibrillation in cats is almost invariably associated with severe structural heart disease that has caused significant enlargement of the atria. Hypertrophic cardiomyopathy is the most common underlying condition, though dilated cardiomyopathy, restrictive cardiomyopathy, and unclassified cardiomyopathies can also predispose cats to this arrhythmia. The enlarged atrial tissue provides the substrate for the abnormal electrical circuits that sustain fibrillation. Unlike in some human patients where atrial fibrillation may occur in otherwise healthy hearts (lone atrial fibrillation), this presentation is extremely rare in cats, making the detection of atrial fibrillation an important indicator that serious cardiac pathology exists.
The impact of atrial fibrillation on affected cats can be substantial, as the arrhythmia itself worsens the cardiac dysfunction already present from underlying heart disease. The loss of effective atrial contraction reduces the heart's filling efficiency by approximately 15-25 percent, which further compromises cardiac output in hearts already struggling with cardiomyopathy. Rapid ventricular rates that often accompany uncontrolled atrial fibrillation put additional strain on the heart muscle and can accelerate the progression of heart failure. The irregular blood flow patterns created by the arrhythmia also increase the risk of blood clot formation within the atria, potentially leading to arterial thromboembolism, one of the most devastating complications of feline heart disease.
Treatment of atrial fibrillation in cats focuses on controlling the ventricular rate to improve cardiac function and managing the underlying heart disease that caused the arrhythmia to develop. Unlike in some human and canine patients, converting cats from atrial fibrillation back to normal sinus rhythm is rarely successful long-term, so treatment typically aims for rate control rather than rhythm control. With appropriate management, some cats with atrial fibrillation can achieve reasonable quality of life, though the presence of this arrhythmia generally indicates advanced cardiac disease and carries important prognostic implications. Regular veterinary monitoring is essential to optimize treatment and detect complications early.
