Brachycephalic airway syndrome is a collection of upper respiratory abnormalities that commonly affect cats with shortened, flattened facial structures, particularly the popular Persian, Himalayan, and Exotic Shorthair breeds. This condition results from the selective breeding that produces the distinctive flat-faced appearance prized in these breeds, which unfortunately compresses and distorts the normal anatomy of the airways. Affected cats experience varying degrees of difficulty breathing, from barely noticeable to severely compromised respiratory function. The syndrome is present from birth and represents one of the most significant health concerns in flat-faced cat breeds.
The anatomical abnormalities that comprise brachycephalic airway syndrome include several components that work together to obstruct normal airflow. Stenotic nares, or narrowed nostrils, restrict air entry at the very beginning of the respiratory tract. An elongated soft palate extends further into the throat than normal, partially blocking the airway during breathing. The nasopharynx, the passage connecting the nasal cavity to the throat, is often narrowed. Some affected cats also have everted laryngeal saccules, where tissue near the larynx is pulled into the airway, and hypoplastic trachea, where the windpipe is narrower than normal. The combination of these defects creates significant resistance to airflow.
The impact of brachycephalic airway syndrome on affected cats ranges from mild inconvenience to life-threatening respiratory compromise. Mildly affected cats may simply breathe more noisily than normal or snore during sleep. Moderately affected cats may have exercise intolerance, difficulty eating without pausing to breathe, and heat intolerance. Severely affected cats struggle to breathe even at rest, may develop cyanosis (blue coloring) during exertion or stress, and are at risk for respiratory crisis and collapse. The syndrome affects quality of life significantly in many cases and can shorten lifespan in severely affected individuals.
Brachycephalic airway syndrome is treatable, with surgical correction offering significant improvement for many affected cats. Procedures to widen the nostrils and shorten the soft palate can substantially improve airflow and quality of life. However, surgery cannot completely normalize the airways in severely affected individuals, and some degree of respiratory compromise may persist. Prevention through responsible breeding practices that prioritize moderate facial conformation offers the best long-term solution for the breed populations. Cat owners considering flat-faced breeds should understand the potential for airway problems and select cats from breeders who prioritize respiratory health.
