The combination of aspirin and clopidogrel, known as dual antiplatelet therapy in human medicine, is occasionally discussed but generally increases bleeding risk without clearly established benefit in cats.

Anticoagulant medications represent a different pharmacological approach to preventing thromboembolism compared to antiplatelet agents like clopidogrel. Low molecular weight heparins such as enoxaparin and dalteparin inhibit clotting factors rather than platelet function and require subcutaneous injection rather than oral administration. While these injectable anticoagulants can be effective, the need for daily or twice-daily injections makes them impractical for long-term home management in many cases. Unfractionated heparin is primarily used in hospital settings for acute management. Newer oral anticoagulants such as rivaroxaban and apixaban have been investigated in cats but have limited published evidence supporting their use and are not yet considered standard therapy for feline thromboembolism prevention.

Beyond antiplatelet and anticoagulant medications, comprehensive management of cats at risk for thromboembolism typically involves addressing the underlying cardiac disease. Medications commonly used alongside clopidogrel in cats with heart disease include atenolol or other beta-blockers to reduce heart rate and improve diastolic filling, diltiazem for similar purposes, and furosemide for cats with congestive heart failure. ACE inhibitors such as benazepril or enalapril may be used in some cardiac patients. Pimobendan has become increasingly used in cats with certain types of cardiomyopathy. These cardiac medications work synergistically with antiplatelet therapy by addressing different aspects of the disease process. Owners should never substitute one cardiac medication for another without veterinary guidance, as each medication has specific indications and the overall treatment plan is carefully designed for each individual patient's needs.
