Plasmoma, also known as atypical pannus or the third eyelid form of chronic superficial keratitis, is an immune-mediated ocular condition in dogs that primarily affects the third eyelid and adjacent conjunctival tissues. This condition is closely related to corneal pannus but targets different structures, causing depigmentation and thickening of the third eyelid margin rather than the characteristic corneal vascularization and pigmentation seen in classic pannus. Plasmoma is characterized by infiltration of plasma cells into the affected tissues, leading to progressive changes that can significantly alter the appearance and function of the third eyelid.
The pathophysiology of plasmoma involves an abnormal immune response where the body's immune system attacks the tissues of the third eyelid. Plasma cells, which are mature B lymphocytes that produce antibodies, accumulate in the third eyelid tissue, causing inflammation and tissue changes. The condition shares the same underlying immune dysfunction as corneal pannus and often occurs concurrently with classic pannus in the same dog. Ultraviolet light exposure appears to play a significant role in triggering and exacerbating the immune response, which explains why the condition is more severe and prevalent at higher altitudes where UV exposure is greater.
The impact of plasmoma on affected dogs can range from mild cosmetic changes to significant functional impairment of the third eyelid. The depigmentation that characterizes the condition makes the leading edge of the third eyelid appear white or pink rather than its normal dark color. Thickening of the third eyelid can interfere with its normal protective function and may cause irritation to the corneal surface. In severe cases, the inflammatory process may extend to involve the cornea, conjunctiva, and other ocular structures. Dogs with plasmoma often have concurrent classic pannus affecting the cornea, creating a more complex clinical picture.
Treatment for plasmoma requires lifelong management with topical immunosuppressive medications to control the immune response and prevent progression. While the condition cannot be cured, consistent treatment can successfully control symptoms and preserve vision and comfort in most affected dogs. Environmental modifications to reduce UV exposure complement medical therapy. Early recognition and treatment provide the best opportunity to prevent significant tissue changes and maintain normal ocular function.
