Pulmonic stenosis is a congenital heart defect in cats characterized by narrowing (stenosis) at or near the pulmonic valve, which controls blood flow from the right ventricle into the pulmonary artery and onward to the lungs. This obstruction increases the resistance against which the right ventricle must pump, creating a pressure overload on the right side of the heart. While pulmonic stenosis is more commonly discussed in dogs, it does occur in cats and represents an important congenital cardiac abnormality that can significantly impact feline health when severe.
The pulmonic valve normally consists of three thin, flexible leaflets that open freely during ventricular contraction to allow blood to flow into the pulmonary artery, then close to prevent backflow. In cats with pulmonic stenosis, various abnormalities can cause obstruction. The most common form involves thickening and fusion of the valve leaflets themselves (valvular stenosis), resulting in a smaller-than-normal opening. Subvalvular stenosis occurs when obstruction is located below the valve, often due to a fibrous ring or muscular hypertrophy. Supravalvular stenosis, involving narrowing above the valve in the pulmonary artery, is less common but can occur.
The impact of pulmonic stenosis on feline health depends on the severity of the obstruction. Mild stenosis may cause minimal hemodynamic disturbance, with affected cats showing no symptoms and living normal lives. Moderate to severe stenosis forces the right ventricle to generate higher-than-normal pressures to push blood through the narrowed opening. This pressure overload leads to thickening (hypertrophy) of the right ventricular wall as the heart muscle adapts to the increased workload. Over time, particularly in severe cases, this compensatory mechanism may fail, leading to right-sided heart failure with fluid accumulation in the abdomen or other body cavities.
Treatment options for pulmonic stenosis include interventional procedures such as balloon valvuloplasty, which can dilate the narrowed valve and reduce the obstruction, as well as surgical approaches for cases not amenable to balloon dilation. Many cats with mild pulmonic stenosis require no treatment and can be monitored. For significant stenosis, intervention can dramatically improve outcomes. The prognosis depends on severity and response to treatment, with many cats achieving good quality of life following appropriate management. Early detection through recognition of the characteristic heart murmur allows timely intervention when needed.
