The causes of glaucoma in cats primarily involve obstruction of the normal aqueous humor outflow pathways. In secondary glaucoma, the most common feline form, inflammation from uveitis causes cellular debris, proteins, and scar tissue to block the drainage angle where fluid exits the eye. Less frequently, primary glaucoma occurs due to inherited anatomical abnormalities of the drainage structures, with certain breeds showing predisposition. Tumors within the eye, lens luxation, and trauma can also obstruct outflow and cause glaucoma. Understanding the underlying cause is critical because treatment must address both the elevated pressure and the primary condition.
The impact of glaucoma on feline health is profound, as the condition is painful and, without treatment, invariably leads to blindness. Elevated intraocular pressure stretches the eye structures, causing discomfort that ranges from chronic headache-like pain to acute severe pain during pressure spikes. The optic nerve, connecting the eye to the brain, suffers progressive damage from the elevated pressure, with nerve cell death occurring within hours to days of uncontrolled pressure elevation. Cats are masters at hiding pain, so advanced disease with significant vision loss may be present before owners recognize a problem. The condition affects quality of life through both direct pain and the disability of vision impairment.
Glaucoma is manageable if detected early, though the condition is often considered incurable in terms of restoring normal eye physiology. Treatment aims to reduce intraocular pressure to levels that prevent or slow further optic nerve damage, preserve remaining vision, and maintain comfort. Medical therapy works for some cats, while others require surgical intervention. Early detection dramatically improves outcomes, as vision lost before treatment cannot be recovered. Veterinary care is essential, as specialized equipment is required to measure intraocular pressure and the condition requires ongoing monitoring and treatment adjustment.
