Chronic kidney disease, also known as chronic renal failure, is a progressive condition characterized by the gradual loss of kidney function over months to years in cats. This condition is one of the most common health problems affecting older cats, with studies suggesting that thirty to forty percent of cats over ten years of age have some degree of kidney dysfunction. Unlike acute kidney injury, which develops suddenly, chronic kidney disease progresses slowly as functional kidney tissue is replaced by scar tissue. The kidneys lose their ability to concentrate urine, filter waste products from the blood, and regulate fluid and electrolyte balance, leading to the accumulation of toxins and metabolic derangements.
Chronic kidney disease develops when the functional units of the kidneys, called nephrons, are damaged and gradually lost over time. Each kidney contains hundreds of thousands of nephrons, and the kidneys have remarkable reserve capacity, meaning significant damage must occur before clinical signs become apparent. By the time chronic kidney disease is diagnosed, typically two-thirds or more of functional nephron mass has already been lost. The causes of initial nephron damage are often unknown, though possibilities include previous infections, toxin exposure, hypertension, and genetic factors. Once damage begins, a cycle of ongoing nephron loss tends to continue even if the original cause is resolved.
The impact of chronic kidney disease on affected cats is substantial and progressive. Early in the disease, cats may show no obvious symptoms as the remaining nephrons compensate for those lost. As the disease progresses, signs of kidney dysfunction emerge, including increased thirst and urination, weight loss, decreased appetite, and vomiting. Advanced disease causes uremia, a toxic buildup of waste products that affects every organ system, leading to neurological symptoms, severe gastrointestinal problems, and overall decline. The condition significantly affects quality of life and life expectancy, though the rate of progression varies considerably between individual cats.
While chronic kidney disease cannot be cured or reversed, it can be effectively managed in many cats, allowing extended survival with good quality of life. Early detection through routine screening, particularly in older cats, enables intervention at early stages when management is most effective. Treatment focuses on slowing disease progression, managing complications, and maintaining quality of life through dietary modification, fluid therapy, and medications targeting specific abnormalities. With dedicated management, many cats with chronic kidney disease live comfortably for months to years after diagnosis, making partnership with veterinary professionals essential for optimal outcomes.
