Hypertensive retinopathy is damage to the retina, the light-sensitive tissue at the back of the eye, caused by chronically elevated blood pressure. In cats, systemic hypertension is a relatively common condition, particularly in older cats and those with underlying diseases such as chronic kidney disease or hyperthyroidism. The delicate blood vessels of the retina are highly susceptible to damage from elevated blood pressure, and the eye is often where the damaging effects of hypertension become most apparent. Hypertensive retinopathy is one of the most significant causes of sudden blindness in older cats and represents a medical emergency requiring prompt intervention.
The condition develops because sustained high blood pressure damages the walls of the small blood vessels throughout the body, with the retinal vessels being particularly vulnerable due to their delicate structure. When these vessels are damaged, they may leak fluid and blood into the surrounding retinal tissue, causing swelling, hemorrhage, and ultimately retinal detachment. The progression from initial vessel damage to complete retinal detachment can occur rapidly, sometimes over days to weeks. Because cats do not typically show obvious signs of discomfort from high blood pressure, the first indication of a problem is often sudden vision loss.
The impact of hypertensive retinopathy on affected cats can be devastating, particularly when vision loss occurs. Cats may suddenly appear disoriented, bump into objects, have dilated pupils that do not respond to light, and seem frightened or confused. Bilateral involvement is common because hypertension affects both eyes simultaneously. Complete blindness from bilateral retinal detachment profoundly affects quality of life, though many cats adapt remarkably well to vision loss over time. Beyond the ocular effects, hypertensive retinopathy signals systemic disease that may also be damaging the kidneys, heart, and brain.
Treatment focuses on rapidly lowering blood pressure to prevent further damage and allowing the best chance for retinal reattachment. Antihypertensive medications, particularly amlodipine, are the mainstay of treatment. When blood pressure is controlled quickly and before extensive structural damage has occurred, some cats regain partial or even complete vision. However, if retinal detachment has been present for more than a few days, permanent vision loss is likely even with successful blood pressure control. Early detection through routine blood pressure monitoring in at-risk cats offers the best opportunity to prevent this devastating complication.
