Diabetes mellitus is strongly associated with candidiasis due to elevated blood glucose providing an excellent growth substrate for yeast and the immune dysfunction that accompanies poorly controlled diabetes. Cats with feline immunodeficiency virus or feline leukemia virus infection have compromised immune systems that make them susceptible to various opportunistic infections including candidiasis. Hyperadrenocorticism (Cushing's disease), though uncommon in cats, causes immunosuppression that predisposes to fungal infections. Cats receiving chemotherapy for cancer or immunosuppressive drugs for immune-mediated conditions face elevated candidiasis risk during treatment. Managing these underlying conditions effectively is essential for controlling and preventing candidiasis.

Conditions with similar symptoms to candidiasis require differentiation for accurate diagnosis and treatment. Bacterial skin infections can produce red, moist, malodorous lesions similar to those of cutaneous candidiasis, and the two may occur concurrently. Dermatophytosis (ringworm) causes skin lesions that may overlap in appearance with candidiasis, though ringworm more commonly produces circular areas of hair loss with scaling at the edges. Ear infections caused by bacteria or ear mites produce similar clinical signs to Candida otitis and may also occur in combination with yeast. Oral lesions from eosinophilic granuloma complex, squamous cell carcinoma, or viral infections may resemble oral candidiasis. Cytology and culture help distinguish between these possibilities and guide appropriate therapy.

Potential complications of candidiasis include secondary bacterial infections that colonize tissues damaged by yeast overgrowth, creating a mixed infection that may be more difficult to treat. Chronic untreated candidiasis can lead to permanent skin changes including scarring, hyperpigmentation, and skin thickening. The most serious complication is dissemination of the infection from superficial sites into the bloodstream and internal organs, leading to systemic candidiasis that can affect the kidneys, liver, eyes, heart, and other vital structures. This progression is most likely in severely immunocompromised cats and represents a life-threatening emergency. Preventing complications through early diagnosis, appropriate treatment, and management of underlying conditions is far preferable to treating advanced disease.
