Nystatin is a polyene antifungal antibiotic that has been used in medicine since the 1950s, primarily for the treatment of Candida yeast infections. In veterinary medicine, nystatin finds application in treating various yeast infections in cats, particularly those involving the gastrointestinal tract, oral cavity, skin, and ears. While Candida infections are relatively uncommon in healthy cats compared to some other species, nystatin provides a valuable treatment option when these opportunistic yeast infections occur, especially in immunocompromised patients or cats with underlying conditions that predispose to fungal overgrowth.
The mechanism of action of nystatin involves binding to ergosterol, a sterol component essential for fungal cell membrane integrity. When nystatin binds to ergosterol, it creates pores in the fungal cell membrane that allow leakage of essential cellular components including potassium and other ions. This disruption of membrane permeability leads to cell death, giving nystatin fungicidal activity against susceptible organisms. Nystatin has a relatively narrow spectrum of activity, being most effective against Candida species and some other yeasts, with limited activity against dermatophytes (organisms causing ringworm) and essentially no antibacterial activity.
Nystatin is available in multiple formulations including oral suspension, tablets, creams, ointments, and as a component of combination ear preparations. A key characteristic of nystatin is that it is not absorbed from the gastrointestinal tract when administered orally, which limits its use to local treatment of surface infections. Oral nystatin acts within the mouth and throughout the gastrointestinal tract to combat yeast in these locations, while topical preparations treat skin and ear infections through direct contact. This lack of systemic absorption contributes to nystatin's excellent safety profile but also means it cannot treat systemic or deep-seated fungal infections.
Nystatin's safety profile in cats is excellent due to the minimal to absent systemic absorption following oral or topical administration. This makes nystatin one of the safer antifungal options available, particularly for treating surface yeast infections in cats that may not tolerate systemic antifungal medications well. However, the limited spectrum of activity and inability to achieve systemic drug levels means that nystatin is not appropriate for all fungal infections, and accurate diagnosis of the causative organism is important for selecting effective treatment. Veterinary guidance ensures proper diagnosis and appropriate use of nystatin when Candida or susceptible yeast infections are identified in feline patients.
