Infective endocarditis is a serious and potentially life-threatening infection of the heart's inner lining, particularly affecting the heart valves. This condition occurs when bacteria or, rarely, fungi colonize damaged or abnormal heart tissue, forming vegetations composed of infectious organisms, platelets, fibrin, and inflammatory cells. While infective endocarditis is relatively uncommon in cats compared to dogs, it carries significant morbidity and mortality when it does occur. The condition typically affects middle-aged cats, with male cats appearing to be more commonly affected than females, though the disease can occur in cats of any age or sex.
The development of infective endocarditis requires two primary factors: a source of bacteremia, or bacteria in the bloodstream, and an abnormal or damaged heart valve surface that provides a site for bacterial attachment. Common sources of bacteremia in cats include dental disease with periodontal infection, skin wounds or abscesses, urinary tract infections, and any invasive procedure or surgery. Pre-existing heart disease, particularly conditions affecting the heart valves such as degenerative valve disease or congenital malformations, creates the damaged endothelial surface that facilitates bacterial colonization. However, some cases occur in cats without known predisposing heart disease, suggesting that bacteremia alone may occasionally be sufficient to initiate infection.
The impact of infective endocarditis on a cat's health is profound and multisystemic. The infection damages heart valves, causing valvular insufficiency that leads to progressive heart failure. Pieces of the vegetations can break off and travel through the bloodstream as septic emboli, lodging in distant organs including the kidneys, spleen, brain, and limbs, causing infarction and spreading infection. The ongoing bacteremia causes systemic inflammatory response with fever, lethargy, and weight loss. Immune complex deposition can damage multiple organs including the kidneys and joints. Without treatment, infective endocarditis is almost universally fatal, and even with aggressive treatment, outcomes remain guarded due to the difficulty of eradicating established cardiac infections.
Despite its serious nature, infective endocarditis can be treated if diagnosed early and managed aggressively. Treatment requires prolonged courses of appropriate antibiotics, typically for at least six to eight weeks, ideally selected based on culture and sensitivity results. Supportive care for heart failure and management of embolic complications are also essential. Prevention focuses on maintaining good dental health, promptly treating infections elsewhere in the body, and using appropriate antibiotic prophylaxis before invasive procedures in high-risk cats. Early recognition of symptoms and prompt veterinary evaluation are critical for achieving the best possible outcome in this challenging condition.
