Valacyclovir is a prodrug of acyclovir, meaning it is converted to acyclovir in the body after administration. While this conversion is intended to improve the bioavailability of acyclovir in humans, in cats the resulting plasma concentrations of acyclovir reach toxic levels that cause devastating multi-organ damage. Research studies in cats experimentally infected with feline herpesvirus type 1 demonstrated that valacyclovir administration resulted in rapid deterioration, with treated cats becoming noticeably more lethargic and dehydrated than controls within six to nine days. The experiments had to be terminated early for humane reasons due to the severity of toxicity observed in treated animals.
The toxicity of valacyclovir in cats is not a matter of dose adjustment or careful monitoring—it is inherent to how cats metabolize this drug. Cats appear to be uniquely sensitive to valacyclovir's toxic effects compared to other species. The hepatic conversion of valacyclovir to acyclovir, combined with the pharmacokinetics of acyclovir in the feline system, produces plasma drug concentrations that are incompatible with survival. Histopathological examination of valacyclovir-treated cats has revealed coagulative necrosis of renal tubular epithelium, centrilobular hepatic atrophy and necrosis, and severe bone marrow depression—a constellation of findings that indicates catastrophic multi-organ failure.

If a cat accidentally ingests valacyclovir or is mistakenly prescribed this medication, immediate veterinary attention is essential. Alternative antiviral medications that are safe and effective for treating feline herpesvirus infections exist and should always be used instead.
