The mechanism of action of pimobendan involves two complementary pathways that work together to improve cardiac function and reduce symptoms of heart failure. First, pimobendan inhibits phosphodiesterase III enzymes in cardiac muscle cells, preventing the breakdown of cyclic adenosine monophosphate and thereby enhancing calcium sensitivity of cardiac contractile proteins. This calcium sensitization increases the force of cardiac contraction without requiring increased calcium influx, which distinguishes pimobendan from other positive inotropes and gives it a favorable safety profile regarding arrhythmia risk. Pimobendan is available as oral chewable tablets or capsules that can be administered at home by pet owners following veterinary instructions, making it practical for long-term management of chronic cardiac conditions. The medication is typically given twice daily and should be administered on an empty stomach, approximately one hour before feeding, to optimize absorption. Many cats accept the chewable tablet formulation readily, though compounded formulations may be prepared for cats requiring alternative dosing forms. The oral route and home administration capability distinguish pimobendan from injectable inotropes that require hospitalization.
The use of pimobendan in cats requires careful case selection and veterinary supervision, as not all forms of feline heart disease benefit equally from this medication. Echocardiographic assessment to characterize the specific type of cardiomyopathy and evaluate cardiac function helps determine appropriate candidates for pimobendan therapy. Cats with systolic dysfunction and heart failure secondary to dilated or end-stage hypertrophic cardiomyopathy are often considered the best candidates, while the use of pimobendan in cats with preserved systolic function remains more controversial.
