Sinusitis of dental origin in horses is a secondary sinus infection that develops when disease from the roots of the upper cheek teeth extends into the adjacent paranasal sinuses. The anatomical relationship between the upper premolars and molars and the maxillary and frontal sinuses creates a pathway for dental infections to spread directly into the sinus cavities. This condition represents one of the most common causes of sinus disease in horses and requires treatment of both the dental source and the resulting sinus infection for successful resolution.
Dental sinusitis affects horses of any breed but is most commonly diagnosed in mature horses whose permanent teeth have fully developed roots in proximity to the sinuses. The upper cheek teeth from the third premolar through the third molar have roots that either directly contact or lie very close to the floor of the maxillary sinus, with the exact relationship varying by tooth position and individual anatomy. Younger horses may be affected when deciduous tooth disease or eruption abnormalities create pathways into the sinus.
The impact of dental sinusitis on equine health extends beyond simple infection to affect respiratory function, comfort, and potentially systemic health. Affected horses typically present with chronic unilateral nasal discharge that may be purulent and malodorous. Facial swelling may develop over the affected sinus. The infection can become chronic if not properly treated, potentially leading to bone involvement, worsening dental disease, and in severe cases, extension to other structures. Horses with dental sinusitis often exhibit signs of discomfort and may have reduced appetite.
Treatment of dental sinusitis requires addressing the underlying dental pathology, which usually means extraction of the affected tooth, combined with treatment of the sinus infection itself. This often involves surgical approaches for sinus access, drainage, and lavage. While dental sinusitis is a serious condition requiring significant intervention, prognosis is generally favorable when properly diagnosed and treated. Early recognition and treatment before extensive bone involvement develops improves outcomes.
