Progressive ethmoid hematoma is an expansile, non-neoplastic mass that originates from the ethmoid labyrinth in the caudal nasal passages of horses. Despite its name containing the word hematoma, this condition is not a simple blood clot but rather a progressive, encapsulated mass of blood products, fibrous tissue, and inflammatory debris that gradually enlarges over time. The mass arises from the highly vascular ethmoid turbinates, which are delicate scroll-like structures in the back of the nasal cavity responsible for warming and filtering inspired air. Once initiated, the lesion continues to grow slowly but persistently, eventually causing significant obstruction of the nasal passages and associated clinical signs.
This condition occurs most commonly in middle-aged to older horses, with the typical age of onset being over nine years old and the majority of cases diagnosed in horses fifteen years of age or older. There does not appear to be a significant breed predisposition, though some studies have suggested slightly higher incidence in Thoroughbreds and Arabians. The condition affects geldings, mares, and stallions with relatively equal frequency. Progressive ethmoid hematoma represents one of the most common causes of unilateral epistaxis, or nosebleeding from a single nostril, in horses, making it an important differential diagnosis for any horse presenting with this clinical sign.
The impact of progressive ethmoid hematoma on equine health depends largely on the size and location of the mass and how long it has been present before diagnosis. Small, early-stage lesions may cause only intermittent mild epistaxis with minimal effect on overall health. However, as the mass enlarges, it progressively obstructs airflow through the affected nasal passage, potentially extending into the paranasal sinuses or even crossing the nasal septum to affect both sides. Large masses can cause significant respiratory obstruction, chronic nasal discharge, facial deformity, and substantial discomfort for the affected horse.
Early detection and treatment of progressive ethmoid hematoma significantly improves outcomes and reduces the likelihood of recurrence. When identified and treated while still small, the condition can often be successfully managed with less invasive procedures and lower recurrence rates than large, advanced lesions. Horse owners should be aware that any unexplained epistaxis, particularly when recurring from the same nostril, warrants veterinary evaluation. With appropriate treatment, many horses with progressive ethmoid hematoma can achieve complete resolution of clinical signs, though long-term monitoring is essential due to the potential for recurrence.
