Unfortunately, hemangiosarcoma carries a much poorer prognosis than benign hematoma, with most dogs surviving only a few months even with treatment. The two conditions share similar clinical presentations, breed predispositions, and age demographics, making differentiation challenging without histopathological examination. This overlap is why definitive diagnosis of splenic masses requires tissue examination following surgical removal, and why the period between surgery and pathology results can be anxious for owners awaiting confirmation of benign versus malignant disease.

Several other conditions may present with similar symptoms to splenic hematoma or affect the same patient population. Nodular hyperplasia, a benign proliferation of splenic tissue, is another common splenic mass in older dogs that may coexist with hematoma. Splenic torsion, though less common, causes acute abdominal pain and shock similarly to ruptured hematoma. Other abdominal tumors or masses may cause similar symptoms of abdominal distension, discomfort, and potential hemorrhage. Immune-mediated hemolytic anemia can cause splenomegaly and may be considered in dogs presenting with anemia and splenic enlargement. Coagulation disorders may predispose to splenic hemorrhage and should be ruled out in appropriate cases.

Potential complications of splenic hematoma and its treatment include acute hemorrhage from rupture, which is the most serious immediate complication. Chronic blood loss leading to anemia may occur with slow bleeding from the hematoma. Post-splenectomy complications can include infection at the surgical site, internal adhesion formation, and rarely, overwhelming post-splenectomy infection. Dogs that have had their spleens removed may have subtle changes in immune function and blood filtering capacity, though most adapt well. Secondary complications from severe anemia or shock at presentation, such as organ damage from poor perfusion, may affect recovery in some patients. Understanding these potential complications helps owners recognize warning signs and seek appropriate veterinary care when needed.
