Sertoli cell tumor is one of the three most common types of testicular cancer affecting intact male dogs, arising from the Sertoli cells that normally support sperm development within the testicles. This tumor type accounts for approximately one-third of all testicular tumors in dogs and is particularly notable for its potential to produce excessive estrogen, leading to a distinctive feminization syndrome. The condition primarily affects older intact males, typically those over seven years of age, though dogs with retained testicles face significantly elevated risk at younger ages. While many Sertoli cell tumors behave in a benign manner, a subset demonstrate malignant behavior with the potential to spread to other organs.
Sertoli cell tumors develop when the supportive Sertoli cells within the testicle undergo abnormal proliferation. These specialized cells normally provide nutrients and structural support to developing sperm cells, but when they become neoplastic, they form masses that can grow to considerable size. The tumor's unique characteristic is its frequent production of estrogen, which circulates throughout the body and causes various secondary effects. Cryptorchidism, the condition where one or both testicles fail to descend into the scrotum, dramatically increases the risk of Sertoli cell tumor development, with retained testicles facing ten to fourteen times higher cancer risk than normally descended testicles.
The impact of Sertoli cell tumors on affected dogs extends beyond the primary tumor itself due to the hormonal disruption these tumors frequently cause. Excessive estrogen production leads to feminization syndrome, characterized by enlarged mammary glands, symmetrical hair loss, skin changes, and attraction of other male dogs. More seriously, sustained high estrogen levels can suppress bone marrow function, potentially causing life-threatening anemia and immune system compromise. The tumor's effects on other body systems may be the first signs owners notice, as the testicular mass itself may not be externally visible, especially in cryptorchid dogs.
Treatment through surgical castration offers excellent outcomes for most dogs with Sertoli cell tumors, particularly when performed before metastasis occurs or severe bone marrow suppression develops. The prognosis is generally favorable for localized tumors, with most dogs returning to normal health following surgery as estrogen levels normalize. However, dogs with metastatic disease or severe estrogen-induced bone marrow suppression face significantly worse outcomes. Early detection through regular veterinary examination of intact males, along with strong recommendations to neuter cryptorchid dogs, provides the best protection against this potentially serious cancer.
