Septic arthritis is a serious and potentially life-threatening joint infection caused by bacteria or, less commonly, fungi or other microorganisms invading the synovial joint space. This condition represents a veterinary emergency because the infectious and inflammatory processes rapidly destroy the delicate articular cartilage lining the joint surfaces, potentially resulting in permanent joint damage within days if not promptly treated. Unlike sterile inflammatory conditions such as immune-mediated arthritis, septic arthritis involves active infection with replicating microorganisms within the joint that must be eliminated through antimicrobial therapy in addition to managing the inflammatory response. The condition can affect any synovial joint but most commonly involves the stifle (knee), elbow, hip, and shoulder joints in dogs.
The pathophysiology of septic arthritis involves bacterial invasion of the joint space followed by rapid proliferation of organisms and an intense inflammatory response. Once bacteria enter the joint, the warm, nutrient-rich synovial fluid provides an ideal growth medium that supports rapid bacterial multiplication. The immune system responds with massive infiltration of white blood cells, primarily neutrophils, that attempt to control the infection through phagocytosis and release of destructive enzymes. Unfortunately, these defensive enzymes also damage the articular cartilage, and the acidic environment created by bacterial metabolism and inflammation further accelerates cartilage breakdown. The synovial membrane becomes thickened and inflamed, joint fluid accumulates, and the normally smooth cartilage surfaces become pitted and eroded.
The impact of septic arthritis on affected dogs is severe and develops rapidly. Dogs typically present with sudden onset of severe lameness, often refusing to bear any weight on the affected limb. The affected joint is markedly swollen, hot, and exquisitely painful to touch or manipulation. Systemic signs of infection including fever, lethargy, and decreased appetite commonly accompany the local joint symptoms. Without prompt treatment, the infection can spread through the bloodstream to affect other joints or organs. Even with appropriate treatment, permanent joint damage may occur if there has been significant cartilage destruction before therapy was initiated, potentially resulting in chronic osteoarthritis and ongoing lameness.
Successful treatment of septic arthritis requires rapid diagnosis followed by aggressive antimicrobial therapy and often surgical intervention to drain the infected joint. The urgency of treatment cannot be overstated, as cartilage damage begins within hours of infection and becomes increasingly difficult to prevent as time passes. Joint fluid analysis confirming bacterial infection guides treatment decisions, and culture results direct selection of appropriate antibiotics. Many cases require surgical lavage (flushing) of the joint to remove accumulated pus, debris, and bacterial products. With prompt, aggressive treatment initiated before extensive damage occurs, many dogs recover good joint function. Delayed or inadequate treatment often results in permanent joint damage requiring ongoing management or salvage procedures.
