Sensitivity to anesthesia in dogs refers to an exaggerated, potentially fatal response to standard doses of anesthetic agents, sedatives, and certain other medications. While any individual dog may have an unusual reaction to anesthesia, the most clinically significant form of this sensitivity is caused by a well-characterized genetic mutation in the MDR1 gene, now formally known as the ABCB1 gene. This mutation results in a defective or absent P-glycoprotein transporter in the blood-brain barrier, allowing drugs that would normally be excluded from the central nervous system to accumulate to toxic levels in the brain.
The condition gained widespread veterinary attention through reports of Collies and related herding breeds experiencing severe neurological depression, prolonged sedation, respiratory failure, and death following routine anesthetic procedures or antiparasitic treatments. Research conducted at Washington State University in the late 1990s and early 2000s identified the specific four-base-pair deletion in the MDR1 gene responsible for these reactions. This discovery transformed veterinary practice by enabling genetic testing and breed-specific anesthetic protocols.
Dogs carrying even one copy of the mutant allele may show increased sensitivity to certain drugs, though dogs homozygous for the mutation (carrying two copies) are at the greatest risk. The condition does not cause any outward symptoms until the affected dog is exposed to a substrate drug. This means owners and veterinarians may be completely unaware of the problem until a crisis occurs during surgery or treatment, making preemptive genetic testing critically important.
Beyond anesthetic agents, the MDR1 mutation affects metabolism of a broad range of drugs including certain antiparasitics, anti-cancer agents, antidiarrheal medications, and immunosuppressants. Understanding this genetic predisposition allows veterinarians to select alternative drugs or use significantly reduced dosages, dramatically lowering the risk of adverse events during any procedure requiring sedation or anesthesia.
