Heartworm disease is a serious and potentially fatal parasitic condition caused by the nematode Dirofilaria immitis. Adult heartworms are long, spaghetti-like roundworms that reside primarily in the pulmonary arteries and, in heavy infections, extend into the right ventricle and right atrium of the heart. The disease is transmitted exclusively through the bite of an infected mosquito, making it a vector-borne illness that cannot be spread directly from dog to dog.
Heartworm disease has been diagnosed in dogs in all fifty United States and throughout many parts of the world, with the highest prevalence historically concentrated in the southeastern United States, the Mississippi River valley, and coastal regions where warm, humid climates support large mosquito populations. However, the geographic range of heartworm disease has expanded over time, and cases are now regularly diagnosed in regions that were previously considered low-risk, including northern states and inland areas.
The significance of heartworm disease lies in its insidious progression and the severe damage it inflicts on the cardiovascular and pulmonary systems. Adult worms living in the pulmonary arteries cause inflammation, thickening, and scarring of the arterial walls, leading to pulmonary hypertension and progressive right-sided heart failure. The longer the infection persists and the greater the worm burden, the more extensive and potentially irreversible the damage becomes.
Despite the severity of heartworm disease, it is one of the most preventable parasitic infections in dogs. Highly effective preventive medications are available that, when administered consistently, provide virtually complete protection against heartworm infection. The disparity between the devastating consequences of untreated heartworm disease and the simplicity and effectiveness of prevention makes heartworm prophylaxis one of the most important aspects of routine canine healthcare.
