Heartworm disease, medically known as dirofilariasis, is a serious and potentially fatal parasitic infection caused by the worm Dirofilaria immitis that affects dogs throughout the world, particularly in areas with significant mosquito populations. This devastating disease occurs when infected mosquitoes transmit microscopic heartworm larvae to dogs during blood feeding, initiating an infection that ultimately results in foot-long worms living within the heart, lungs, and associated blood vessels. Heartworm disease represents one of the most significant parasitic threats to canine health, with infections reported in all fifty United States and in temperate and tropical regions worldwide.
The pathophysiology of heartworm disease involves a complex lifecycle requiring both mosquito vectors and canine hosts for completion. When an infected mosquito bites a dog, it deposits heartworm larvae into the skin, where they begin migrating through body tissues toward the heart and pulmonary arteries over a period of approximately six months. Upon reaching the cardiovascular system, the larvae mature into adult worms that can grow twelve inches or longer and live for five to seven years, continuously producing offspring called microfilariae that circulate in the bloodstream awaiting pickup by feeding mosquitoes.
The impact of heartworm infection on canine health ranges from minimal initial effects to severe, life-threatening cardiovascular compromise depending on the worm burden and duration of infection. Adult heartworms cause progressive damage to the heart, lungs, and blood vessels through both their physical presence and inflammatory reactions they trigger. Dogs with heavy worm burdens may develop heart failure, severe lung disease, and damage to other organs including the liver and kidneys. Without treatment, heartworm disease is almost always fatal, making prevention and early detection absolutely critical.
Heartworm disease is both preventable and treatable, though treatment carries significant risks and is far more expensive and complicated than prevention. Monthly preventive medications are highly effective at stopping infection before it becomes established, representing the gold standard approach to heartworm disease management. When prevention fails and infection occurs, treatment protocols involving injectable medications can eliminate adult worms, though the treatment process requires careful management and extended exercise restriction.
