Renal failure in dogs refers to the inability of the kidneys to adequately perform their essential functions of filtering metabolic waste products from the blood, regulating fluid and electrolyte balance, maintaining acid-base homeostasis, and producing critical hormones including erythropoietin and calcitriol. The kidneys are remarkably resilient organs with substantial functional reserve capacity, and clinical signs of failure typically do not become apparent until approximately 66 to 75 percent of functional kidney tissue has been lost. This significant reserve means that by the time renal failure is diagnosed, the disease process is often well advanced.
Renal failure is broadly classified into two categories based on the speed of onset and the underlying pathological process. Acute kidney injury, formerly referred to as acute renal failure, develops rapidly over hours to days and may be reversible with prompt and appropriate treatment. Chronic kidney disease develops gradually over months to years as functional nephrons are progressively and irreversibly destroyed. While these two forms share certain clinical features, they differ fundamentally in their causes, management approaches, and prognoses.
The prevalence of chronic kidney disease increases significantly with age, making it one of the most common medical conditions diagnosed in senior and geriatric dogs. Studies estimate that chronic kidney disease affects approximately one in ten dogs over the age of seven, with prevalence rising further in dogs over twelve years of age. Acute kidney injury, while less common overall, can affect dogs of any age and often results from identifiable inciting causes such as toxin ingestion, infectious disease, or ischemic events.
Understanding the distinction between kidney disease and kidney failure is important for both veterinary professionals and dog owners. Kidney disease encompasses any pathological process affecting the kidneys, while kidney failure specifically denotes the point at which the kidneys can no longer maintain normal homeostasis. A dog may live with compensated kidney disease for an extended period before progressing to overt failure, and early identification of kidney disease through routine screening provides the best opportunity for intervention and slowing of disease progression.
