Protein-losing enteropathy is a syndrome in dogs characterized by the excessive loss of protein through the gastrointestinal tract, resulting in abnormally low blood protein levels that produce wide-ranging systemic effects. Rather than a single disease, PLE represents a group of conditions that share the common feature of intestinal protein loss. The lost proteins include albumin, which maintains fluid balance and transports numerous substances through the bloodstream, and globulins, which include antibodies essential for immune function. When protein loss exceeds the liver's ability to produce replacement proteins, blood protein levels decline progressively, leading to fluid accumulation, immune compromise, and eventually life-threatening complications.
The underlying causes of protein-losing enteropathy include several distinct intestinal conditions that damage or dysfunction the intestinal barrier. Inflammatory bowel disease represents one of the most common causes, as chronic inflammation damages the intestinal lining and allows proteins to leak into the intestinal contents. Intestinal lymphangiectasia involves abnormal dilation of lymphatic vessels that leak protein-rich lymph fluid. Intestinal lymphoma and other cancers may infiltrate the intestinal wall and cause protein leakage. Severe parasitic or infectious enteritis can also produce PLE. Identifying the specific underlying cause is essential for developing effective treatment strategies.
The impact of protein-losing enteropathy on affected dogs extends far beyond the gastrointestinal tract. Low albumin levels reduce the oncotic pressure that keeps fluid within blood vessels, causing fluid to leak into body cavities and tissues. This results in abdominal swelling from ascites, limb swelling from peripheral edema, and potentially respiratory difficulty from chest effusion. Loss of immune globulins compromises the dog's ability to fight infections. Antithrombin III loss increases the risk of dangerous blood clots. The chronic nature of PLE means that dogs often present with advanced systemic effects when finally diagnosed.
Protein-losing enteropathy requires lifelong management, but many affected dogs can achieve good quality of life with appropriate treatment. Early diagnosis, before severe protein depletion develops, offers the best prognosis. Treatment typically involves dietary modification, particularly ultra-low-fat feeding, combined with medications that address the underlying cause and reduce intestinal inflammation. Regular monitoring of blood protein levels guides treatment adjustments. While complete cure is rarely achieved, effective management can control symptoms, restore blood protein levels toward normal, and allow dogs to enjoy active, comfortable lives for extended periods.
