Pyelonephritis is a bacterial infection of the kidney that occurs when bacteria ascend from the lower urinary tract or, less commonly, spread through the bloodstream to infect kidney tissue. This serious condition affects the kidney parenchyma (functional tissue) and the renal pelvis, the funnel-shaped structure that collects urine before it drains into the ureter. Pyelonephritis can occur as an acute infection with sudden severe illness or as a chronic condition with persistent low-grade infection causing progressive kidney damage. The condition can affect one or both kidneys and, if left untreated, can lead to permanent kidney damage, sepsis, or kidney failure.
Bacterial invasion of the kidneys typically occurs through ascending infection from the bladder, where bacteria travel up the ureters against the normal flow of urine. Escherichia coli (E. coli) is the most common causative organism, accounting for the majority of cases, though other bacteria including Staphylococcus, Streptococcus, Proteus, Klebsiella, Enterococcus, and Pseudomonas species can also cause pyelonephritis. Conditions that promote lower urinary tract infections or impair normal urinary defenses increase the risk of ascending infection. Once bacteria reach the kidney, they multiply within the tissue, triggering an intense inflammatory response that damages kidney structures.
The impact of pyelonephritis on affected dogs ranges from mild illness to life-threatening systemic infection depending on the severity and whether treatment is initiated promptly. Acute pyelonephritis typically causes fever, pain, lethargy, and signs of systemic illness as the body responds to infection. The affected kidney becomes inflamed, swollen, and painful. If infection is severe or bilateral, kidney function may be significantly impaired. Chronic pyelonephritis causes ongoing low-grade inflammation that gradually destroys kidney tissue, potentially leading to chronic kidney disease. Scarring from repeated or persistent infection permanently reduces kidney function.
Pyelonephritis is a treatable condition when diagnosed and managed appropriately. Antibiotic therapy targeting the causative bacteria forms the cornerstone of treatment, with selection based on culture and sensitivity testing to ensure effectiveness. Treatment duration for kidney infections is substantially longer than for simple bladder infections due to the difficulty of achieving adequate antibiotic concentrations in kidney tissue. Supportive care including fluid therapy addresses dehydration and supports kidney function. Identifying and correcting predisposing factors reduces recurrence risk. With appropriate treatment, most dogs recover from acute pyelonephritis, though monitoring for kidney damage is important. Chronic pyelonephritis requires prolonged management and may result in permanent kidney impairment despite treatment.
