Under normal circumstances, a small amount of fluid constantly filters from pulmonary capillaries into the interstitial spaces but is efficiently removed by lymphatic drainage. Pulmonary edema develops when this balance is disrupted, either through increased hydrostatic pressure forcing more fluid out of vessels, decreased oncotic pressure reducing the forces keeping fluid in vessels, increased capillary permeability allowing fluid to leak more freely, or impaired lymphatic drainage. The accumulated fluid fills alveoli and impairs oxygen transfer, producing the characteristic respiratory distress.
The impact of pulmonary edema on affected dogs is profound and potentially life-threatening. As fluid accumulates in the lungs, the effective surface area for gas exchange decreases dramatically. Dogs must work increasingly hard to move air through fluid-filled tissues, leading to rapid, labored breathing. Oxygen levels in the blood fall while carbon dioxide may accumulate, affecting all body systems that depend on adequate oxygenation. The sensation of being unable to breathe adequately causes significant distress and anxiety. Diuretic medications help remove excess fluid from the body, reducing pulmonary congestion. Oxygen supplementation supports gas exchange while other treatments take effect. When pulmonary edema results from heart disease, which is the most common cause in dogs, specific cardiac medications become essential components of treatment.
