Proptosis is a serious ocular emergency in which the eyeball becomes displaced forward out of the eye socket, with the eyelids closing behind it and trapping the globe in an abnormal anterior position. This traumatic condition occurs when significant force is applied to the head or neck, causing the eye to pop out beyond the confines of the bony orbit that normally protects and contains it. Proptosis constitutes a true veterinary emergency requiring immediate professional intervention to save the eye and preserve any potential for vision.
The condition occurs when external trauma or pressure causes the eyeball to displace anteriorly through the palpebral fissure, the opening between the eyelids. Once the widest part of the globe passes beyond the eyelid margins, the lids snap closed behind the eye, holding it in the proptosed position. This entrapment prevents spontaneous return of the eye to its normal position and begins a cascade of damaging effects including corneal exposure, restricted blood supply, and tension on the optic nerve.
Brachycephalic breeds with their characteristically shallow orbits and prominent eyes are dramatically more susceptible to proptosis than dogs with normal skull anatomy. Breeds such as Pugs, Boston Terriers, Shih Tzus, Pekingese, and Cavalier King Charles Spaniels can experience proptosis from relatively minor trauma that would not affect dogs with deeper eye sockets. In contrast, dogs with normal or deep-set eyes typically require severe trauma to cause proptosis, and such cases often have more associated injuries and worse prognoses.
Time is critical in proptosis cases because prolonged displacement leads to progressive and potentially irreversible damage. The cornea dries and deteriorates without the protection of the eyelids and tear film. Blood supply to the eye may be compromised, causing tissue death. The optic nerve sustains stretch injury that threatens vision even if the eye is successfully replaced. Seeking emergency veterinary care immediately upon recognizing proptosis provides the best chance for saving the eye and any vision.
