Pemphigus vulgaris is a rare but severe autoimmune blistering disease in dogs characterized by deep erosions and ulcerations affecting primarily the mucous membranes and mucocutaneous junctions. Unlike the more common pemphigus foliaceus, which affects superficial skin layers, pemphigus vulgaris targets deeper layers of the epidermis, resulting in more severe and potentially life-threatening disease. The oral cavity is most commonly and severely affected, causing painful ulcerations that significantly impact the dog's ability to eat and drink. This condition requires aggressive immunosuppressive treatment and carries a more guarded prognosis than other pemphigus variants.
The underlying mechanism of pemphigus vulgaris involves autoantibodies targeting desmoglein-3, a cadherin protein essential for cell adhesion in the deeper layers of the epidermis and in mucosal tissues. When these antibodies bind to desmoglein-3, they disrupt the connections between keratinocytes, causing cells to separate from one another in a process called acantholysis. Because the cleavage occurs deep within the epidermis, just above the basal cell layer, the resulting blisters are more fragile and rapidly rupture to form painful erosions. The preference of desmoglein-3 for mucosal tissues explains why oral lesions are so prominent in this disease.
Dogs with pemphigus vulgaris typically present with severe oral ulcerations, drooling, reluctance to eat, and weight loss. Lesions commonly affect the gums, lips, tongue, and palate, causing significant pain and difficulty eating. The disease may extend to other mucocutaneous areas including the nose, eyelids, vulva, prepuce, and anus. Skin involvement, when present, produces fragile blisters that quickly rupture into raw, weeping erosions rather than the thick crusts seen in pemphigus foliaceus. The severity of mucosal involvement often leads to systemic illness with fever, dehydration, and secondary infections.
Treatment of pemphigus vulgaris requires aggressive immunosuppressive therapy, typically combining high-dose corticosteroids with additional immunosuppressive medications. The treatment approach must balance disease control against significant medication side effects. Even with optimal treatment, pemphigus vulgaris carries a more guarded prognosis than pemphigus foliaceus, and some dogs may not achieve adequate disease control. However, with committed treatment and close veterinary monitoring, some dogs do achieve remission and can maintain reasonable quality of life. The severity of this condition necessitates working with veterinary specialists experienced in managing severe autoimmune diseases.
