Oleander toxicity is an extremely serious and potentially fatal poisoning that occurs when dogs ingest any part of the oleander plant, scientifically known as Nerium oleander. This attractive evergreen shrub, popular in landscaping throughout warmer climates, is considered one of the most toxic plants commonly found in residential areas. Every part of the oleander plant is highly poisonous, including the flowers, leaves, stems, roots, and even the smoke from burning the plant. The toxins can also contaminate water where oleander branches have been placed or soil where the plant grows.
Oleander contains multiple cardiac glycosides, with oleandrin being the most significant toxic compound. These substances exert powerful effects on the heart, disrupting the electrical conduction system and the contractile function of cardiac muscle. The mechanism is similar to that of digitalis from foxglove, but oleander is considerably more toxic on a weight basis. The cardiac glycosides in oleander inhibit the sodium-potassium ATPase pump in heart cells, leading to dangerous accumulations of calcium that destabilize cardiac rhythm and can cause fatal arrhythmias.
The clinical presentation of oleander poisoning typically begins with gastrointestinal symptoms including vomiting, diarrhea, and abdominal pain, often developing within hours of ingestion. As the cardiac glycosides are absorbed into the bloodstream and reach the heart, life-threatening arrhythmias can develop rapidly. Dogs may show weakness, collapse, or sudden death with minimal warning. The progression from mild symptoms to cardiac crisis can occur within hours, making immediate veterinary intervention essential for any suspected oleander exposure.
Despite its extreme toxicity, dogs that receive rapid aggressive treatment may survive oleander poisoning. Treatment focuses on preventing further toxin absorption, stabilizing cardiac rhythm, and providing intensive supportive care until the glycosides are eliminated from the body. The antidote used in human medicine, digoxin-specific antibody fragments, can be highly effective when available. Given the plant's widespread presence in many communities and its exceptional toxicity, dog owners in regions where oleander is grown should be familiar with this plant and ensure their pets cannot access it.
