Common primary cancers that spread to the lungs include osteosarcoma, which has extremely high pulmonary metastatic rates; mammary carcinoma, particularly high-grade tumors; hemangiosarcoma, which typically has widespread metastasis at diagnosis; oral melanoma; thyroid carcinoma; and various soft tissue sarcomas. Understanding the biology and behavior of the primary cancer is essential for managing metastatic disease, as treatment options and prognosis are heavily influenced by the primary tumor type. Staging evaluation for metastases in other organs beyond the lungs is typically performed when pulmonary metastases are identified.

Primary lung tumors must be differentiated from metastatic lung disease, as treatment approaches and prognosis differ. Primary lung adenocarcinoma, bronchogenic carcinoma, and other primary pulmonary cancers typically present as single lung masses rather than multiple nodules. Multiple pulmonary nodules are more suggestive of metastatic disease and warrant investigation for a primary tumor elsewhere. However, some primary lung cancers can present with multiple nodules, and definitive differentiation may require biopsy. In dogs without known primary cancers presenting with lung masses, thorough diagnostic evaluation seeks to determine whether the lung is the primary site or metastatic destination.

Potential complications of metastatic lung disease include progressive respiratory failure as tumor burden increases, pleural effusion from metastatic spread to pleural surfaces, pneumothorax if tumors erode through the pleural surface, pulmonary hemorrhage if tumors invade blood vessels, and cachexia and declining body condition from advanced cancer. Spread to additional organs beyond the lungs may occur, with common secondary metastatic sites including liver, lymph nodes, bone, brain, and adrenal glands depending on primary tumor type. Recognition of these complications guides appropriate supportive care and helps inform discussions about prognosis and quality of life considerations throughout the disease course.
