Hydronephrosis is a condition in dogs characterized by swelling and dilation of the kidney due to the accumulation of urine within the renal pelvis and calyces, the internal collecting structures of the kidney. This occurs when urine cannot flow normally from the kidney through the ureter to the bladder, causing a backup of fluid that progressively distends the kidney. The underlying cause is typically some form of obstruction in the urinary tract, though it can also result from abnormal anatomy present since birth. Left untreated, the pressure from accumulated urine damages the delicate kidney tissue and can eventually destroy kidney function entirely.
The mechanism of hydronephrosis involves increased pressure within the kidney's collecting system when urine outflow is blocked or severely restricted. Under normal conditions, urine produced by the kidney flows continuously through the renal pelvis into the ureter and down to the bladder. When this flow is impeded, urine backs up and the relatively elastic renal pelvis begins to expand. As pressure rises, it transmits to the functional kidney tissue, compressing the delicate structures responsible for filtering blood and producing urine. Initially, this damage may be reversible if the obstruction is relieved, but prolonged hydronephrosis leads to permanent loss of kidney function through pressure atrophy and ischemic damage.
The impact of hydronephrosis on affected dogs depends heavily on whether one or both kidneys are involved and the duration of the obstruction. Unilateral hydronephrosis, affecting only one kidney, may cause minimal symptoms initially because the healthy opposite kidney compensates effectively. Dogs with bilateral hydronephrosis or those with pre-existing kidney disease are at much greater risk of developing life-threatening kidney failure. The degree of kidney damage correlates with both the severity and duration of obstruction, making early detection and treatment critical for preserving kidney function.
Treatment for hydronephrosis focuses on relieving the obstruction to restore normal urine flow and preserve as much kidney function as possible. Depending on the underlying cause, this may involve surgical removal of stones, correction of anatomical abnormalities, or placement of devices to bypass the obstruction. Some cases require removal of the affected kidney if function has been irreversibly lost. Prompt veterinary attention when hydronephrosis is suspected significantly improves the chances of preserving kidney function and achieving a good outcome. Dogs who receive appropriate treatment before permanent damage occurs often recover well and can live normal lives.
