Hydrocephalus is a neurological condition characterized by abnormal accumulation of cerebrospinal fluid within the brain's ventricular system, causing ventricular enlargement and increased pressure on brain tissue. Often referred to as water on the brain, this condition predominantly affects toy and small breed dogs, though it can occur in any breed. Congenital hydrocephalus, present from birth, is the most common form seen in predisposed breeds, while acquired hydrocephalus may develop later in life secondary to other conditions. The brain normally produces cerebrospinal fluid continuously within specialized structures called the choroid plexus located in the brain's ventricles. This fluid circulates through the ventricular system, bathes the brain and spinal cord, and is absorbed back into the bloodstream through structures overlying the brain. When production exceeds absorption or when flow is obstructed, cerebrospinal fluid accumulates within the ventricles. The resulting pressure causes ventricular dilation and compression of surrounding brain tissue, producing the neurological signs associated with hydrocephalus.
The impact of hydrocephalus on affected dogs varies tremendously based on the degree of ventricular enlargement and the areas of brain tissue affected. Some dogs with mild hydrocephalus live relatively normal lives with minimal or no symptoms. Others experience severe cognitive impairment, vision loss, seizures, or motor dysfunction that significantly affects quality of life. The developing brains of young puppies may accommodate moderate ventricular enlargement without obvious symptoms, while rapid fluid accumulation in older dogs often produces more acute signs.
Treatment options for hydrocephalus include medical management to reduce cerebrospinal fluid production and surgical placement of shunt systems to divert excess fluid. The choice of treatment depends on symptom severity, rate of progression, and individual patient factors. Many dogs with mild to moderate hydrocephalus respond well to medical management, while those with severe or rapidly progressive disease often require surgical intervention.
