Dirofilaria immitis is a filarial nematode parasite responsible for heartworm disease, one of the most serious and potentially fatal conditions affecting domestic dogs worldwide. The adult worms reside primarily within the pulmonary arteries and, in heavy infections, extend into the right ventricle of the heart. These parasites can grow to impressive lengths, with females reaching 25 to 31 centimeters and males reaching 12 to 20 centimeters. A single infected dog may harbor anywhere from one to over 250 adult worms, though typical worm burdens in untreated dogs range from 15 to 50.
The disease is classified as a vector-borne parasitic infection because it requires a mosquito intermediate host to complete its lifecycle. More than 70 species of mosquitoes are capable of transmitting Dirofilaria immitis, making geographic avoidance virtually impossible in endemic areas. The parasite cannot be transmitted directly from dog to dog, and the mosquito plays an essential biological role in the maturation of the larval stages rather than simply acting as a mechanical carrier.
Heartworm disease has been documented on every continent except Antarctica, though its prevalence is highest in regions with warm temperatures, high humidity, and large mosquito populations. In the United States, the disease was historically concentrated in the southeastern states and along the Mississippi River, but cases are now reported in all 50 states. Climate change, wildlife reservoirs such as coyotes and foxes, and the transportation of infected dogs across regions have contributed to the expanding geographic range of the parasite.
The economic and emotional toll of heartworm disease is substantial. Treatment of an established infection is expensive, carries inherent medical risks, and requires prolonged exercise restriction. Prevention, by contrast, is safe, affordable, and highly effective. Despite the availability of excellent preventive medications, millions of dogs remain unprotected, and heartworm disease continues to be diagnosed at alarming rates across veterinary practices.
