Heart valve dysplasia is a congenital cardiac condition in which one or more of the heart's valves develop abnormally during fetal growth, resulting in structural malformations that impair normal valve function. The heart contains four valves that regulate blood flow through its chambers: the mitral valve between the left atrium and left ventricle, the tricuspid valve between the right atrium and right ventricle, and the aortic and pulmonic semilunar valves guarding the outflow tracts. Dysplasia most commonly affects the atrioventricular valves, with tricuspid valve dysplasia and mitral valve dysplasia being the predominant forms encountered in veterinary cardiology.
The malformation can involve the valve leaflets themselves, the chordae tendineae that anchor the leaflets to the papillary muscles, the papillary muscles within the ventricle, or any combination of these structures. When these components are malformed, the valve cannot close properly during cardiac contraction, allowing blood to flow backward through the incompetent valve. This backward flow, known as valvular regurgitation, forces the heart to work harder to maintain adequate cardiac output, leading to progressive chamber enlargement and eventual heart failure if the condition is severe.
Heart valve dysplasia is distinguished from acquired valvular disease, such as myxomatous mitral valve disease, which develops later in life due to degenerative changes in previously normal valve tissue. Dysplastic valves are structurally abnormal from birth, though clinical signs may not become apparent until the dog matures and the hemodynamic consequences of the malformation become more pronounced.
The severity of heart valve dysplasia ranges widely among affected dogs. Some individuals have mild valve abnormalities that cause a detectable heart murmur but produce no clinical symptoms throughout life, while others have severely malformed valves that lead to profound regurgitation, rapid cardiac deterioration, and congestive heart failure at a young age. This spectrum of severity underscores the importance of thorough cardiac evaluation in breeds known to carry this condition.
