Bronchiectasis is a chronic respiratory condition characterized by permanent, irreversible dilation and structural damage to the bronchi, the medium-sized airways that branch from the trachea to deliver air throughout the lungs. This pathological widening of the airways results from destruction of the elastic and muscular components of the bronchial walls, leaving them weakened, distorted, and unable to function normally. The dilated bronchi lose their ability to effectively clear mucus and debris, creating a self-perpetuating cycle of mucus accumulation, bacterial colonization, recurrent infection, and progressive airway damage. Unlike acute respiratory infections that resolve with treatment, bronchiectasis represents permanent structural changes that require lifelong management to control symptoms and slow disease progression.
The development of bronchiectasis follows a pattern known as the vicious cycle hypothesis, where initial airway injury from infection, inflammation, or structural abnormality impairs normal clearance mechanisms. This impairment leads to mucus stasis, which promotes bacterial growth and chronic infection. The ongoing infection and associated inflammatory response cause further damage to the airway walls, worsening the dilation and clearance dysfunction. This cycle continues unless interrupted by treatment, progressively expanding the extent and severity of airway involvement. Understanding this pathophysiology guides treatment approaches that target multiple points in the cycle including infection control, inflammation reduction, and mucus clearance enhancement.
Bronchiectasis in dogs can develop as a consequence of various underlying conditions or may be present from birth in dogs with primary ciliary dyskinesia, a genetic disorder affecting the cilia that normally sweep mucus out of the airways. Acquired bronchiectasis most commonly results from severe or repeated respiratory infections, particularly those occurring in young dogs before their airways have fully developed. Chronic inflammatory conditions, foreign body aspiration with prolonged airway obstruction, smoke inhalation injury, and immune deficiencies can all lead to the bronchial damage that results in bronchiectasis. The condition may be localized to specific lung lobes when resulting from focal injury or infection, or diffuse when caused by systemic conditions like ciliary dyskinesia.
The clinical significance of bronchiectasis lies in its chronic nature and the ongoing management challenges it presents for affected dogs and their owners. Dogs with bronchiectasis typically experience persistent or recurrent coughing, exercise intolerance, and intermittent respiratory infections that can significantly impact quality of life. While bronchiectasis cannot be cured, appropriate management can substantially improve symptoms, reduce infection frequency, and slow progression of airway damage. Working closely with a veterinarian to develop and maintain an individualized management plan helps dogs with bronchiectasis live more comfortable lives despite their chronic condition.
